We gladly accept referrals from your physician or health provider. Please have them fax the referral, with pertinent notes and workup to:

We will also see patients without a referral if we can help you with your problem.

Dr. Buchanan will review the new patient forms and accept you as a patient if he feels he can help you. If you have seen a neurologist in the past, or had brain imaging, labs, or other testing for this problem in the past, please have these records sent to our office.

Once we have the information above, we will schedule you for a New Patient Visit.

We require the following paperwork to be filled out completely and returned to us. Please fill out the correct paperwork below based on the problems you are having:

• Fill out this paperwork if being seen for seizures, spells, or blackouts.
• Fill out this paperwork if being seen for headaches or facial pain.
• Fill out this paperwork if being seen for problems with memory.
• Fill out this paperwork if being seen for Multiple Sclerosis.
• Fill out this paperwork if being seen for any other neurological problem.

Once we have received your completed paperwork, we will contact you to schedule an appointment with Dr. Buchanan or with Heather Bascom.

At High Desert Neurology, we strive to see patients promptly. You may be scheduled to see Dr. Buchanan or Heather Bascom. Heather Bascom and Dr. Buchanan work closely on all patients and review the history, tests and imaging, and the plan for each patient.
