Becoming a New Patient
Step One
The initial step is obtaining a referral from your healthcare provider. Simply have your doctor fax it to us at 855-933-2297. Our staff will review the paperwork and schedule the appointment.
Step Two
After your referral has been sent, fill out the appropriate paperwork:
